Carvykti vs Tecvayli: how two US-approved medicines for multiple myeloma compare

A side-by-side guide for families in the Gulf, built from the US FDA labels, Gulf drug registers and official sources.

Reviewed by the Reserve Meds clinical operations team on 17 October 2026. Facts on this page come from the current US prescribing information for each medicine.

Carvykti vs Tecvayli: the short answer

Carvykti and Tecvayli are both US FDA-approved for multiple myeloma, but for different patients and in different ways. No trial has compared them head to head. Your specialist decides which, if either, fits. Reserve Meds is a digital health company. We do not diagnose or prescribe.

  • Condition: multiple myeloma.
  • Main difference: Both can be used after one prior line of therapy, but for different patients.
  • Head-to-head evidence: none in either US label.

About multiple myeloma and these two medicines

Multiple myeloma is a cancer that begins in plasma cells, a type of white blood cell that is part of the immune system. In time, myeloma cells collect in the bone marrow and in the solid parts of bones.[1]

Carvykti and Tecvayli both target BCMA, a protein on myeloma cells, but in different ways. Carvykti is a CAR T-cell therapy made from the patient's own T cells and given once by infusion. Tecvayli is a bispecific antibody, not made from the patient's cells, injected under the skin on a repeating schedule.[2, 3, 4]

The main difference

Both can be used after one prior line of therapy, but for different patients. Carvykti is for disease that is refractory to lenalidomide after at least one prior line. Tecvayli after one prior line is given with daratumumab and hyaluronidase-fihj; used alone, it is for patients after at least four prior lines. Carvykti is a single infusion at a qualified center; Tecvayli is an ongoing injection schedule.[3, 4]

The evidence behind each approval

Carvykti. CARTITUDE-4 was a randomized, open-label study in 419 adults with lenalidomide-refractory myeloma after at least one prior line. It compared Carvykti with standard therapy (DPd or PVd). The primary measure was progression-free survival, and the label reports a statistically significant improvement in overall survival at a prespecified interim analysis. CARTITUDE-1 was an earlier single-arm study.[3]

Tecvayli. MajesTEC-3 was a randomized, open-label study in 587 patients after one to three prior lines. Tecvayli with daratumumab and hyaluronidase-fihj was compared with DPd or DVd, and the label reports a statistically significant improvement in progression-free survival and overall survival. MajesTEC-1 was a single-arm study of Tecvayli alone in 110 patients, based on overall response rate.[4]

No head-to-head trial. Neither US label reports a study that compared Carvykti directly with Tecvayli. The trials above enrolled different patients at different stages, so their results cannot be set side by side to decide which medicine works better.

Using one after the other

Neither label reports a study of one after the other, and the MajesTEC-3 trial of Tecvayli excluded patients who had received any prior BCMA-directed therapy. Your specialist decides the order of treatments.[3, 4]

Safety: what the labels warn about

Both labels list warnings that the treating team monitors. The boxed warning row in the table above is the most serious level of warning on a US label.

  • Carvykti: common side effects listed on the label include pyrexia (fever), cytokine release syndrome, low antibody levels (hypogammaglobulinemia), low blood pressure, musculoskeletal pain and fatigue.[3]
  • Tecvayli: common side effects listed on the label include fever, cytokine release syndrome, musculoskeletal pain, injection site reactions, fatigue, upper respiratory tract infection, nausea and headache (when used alone).[4]

How families in the Gulf can access Carvykti or Tecvayli

If the medicine is listed in your country. Ask your specialist and the hospital pharmacy about local supply first. A listing does not mean every hospital stocks it, but local supply is usually the fastest route and you may not need us.

If it is not registered. The usual legal route is a named-patient request: the treating hospital, on your specialist's written request, asks the national regulator for permission to import the medicine for one patient, using the US FDA approval as the reference. In Saudi Arabia the SFDA Special Access Program guidance covers these requests; in Bahrain, NHRA Circular 41 of 2020 sets the procedure. Other Gulf countries have their own procedures, which the hospital pharmacy confirms.[7, 8] Once approval is in place, Reserve Meds arranges supply through a US-licensed pharmaceutical wholesale partner to the hospital pharmacy.

For Carvykti. CAR T-cell therapy is made from the patient's own cells, so treatment happens at a qualified treatment center that can collect the cells, give the lymphodepleting chemotherapy and manage side effects. For most Gulf families that means a referral from the treating specialist. A local hospital can only give it if the manufacturer agrees to supply that hospital and the regulator approves. We cannot promise that a manufacturer will agree.

A US safety program applies. In the US, Tecvayli is available only through the TECVAYLI and TALVEY REMS, a restricted program.[4] The treating hospital must be able to give the step-up doses with hospital monitoring as the label describes.

We give you a written timeline estimate once we see the hospital file. The regulator, the treatment center and the manufacturer set their own timelines.
