A side-by-side guide for families in the Gulf, built from the US FDA labels, Gulf drug registers and official sources.

Itovebi vs Orserdu: the short answer

About advanced breast cancer and these two medicines

Breast cancer is a cancer that starts in breast tissue. It happens when cells in the breast change and grow out of control. If the cancer spreads to other parts of the body through the lymph system or the blood, it is called metastatic.[1]

Itovebi and Orserdu are both for HER2-negative advanced breast cancer that is hormone receptor-positive (Itovebi label) or estrogen receptor-positive (Orserdu label), and both need a specific gene change found by a blood test: a PIK3CA mutation for Itovebi and an ESR1 mutation for Orserdu. The labels name FDA-approved or FDA-authorized tests for this.[2, 3]

The main difference

They are for different gene changes and different points in treatment. Itovebi is for PIK3CA-mutated disease that came back during or after adjuvant hormone therapy, and it is taken with palbociclib and fulvestrant. Orserdu is for ESR1-mutated disease that progressed after at least one line of hormone therapy, and it is taken alone.[2, 3]

The evidence behind each approval

Itovebi. INAVO120 was a randomized, double-blind, placebo-controlled trial in 325 patients. Itovebi or placebo was added to palbociclib and fulvestrant, and the major efficacy outcome was progression-free survival.[2]

Orserdu. EMERALD was a randomized, open-label trial in 478 patients, 228 of whom had ESR1 mutations. It compared Orserdu with the investigator's choice of fulvestrant or an aromatase inhibitor, and the major efficacy outcome was progression-free survival.[3]

No head-to-head trial. Neither US label reports a study that compared Itovebi directly with Orserdu. The trials above enrolled different patients at different stages, so their results cannot be set side by side to decide which medicine works better.

Using one after the other

The tests look for each gene change separately. Neither label reports a study of the two medicines together or one after the other. Your oncologist decides based on the test results and earlier treatment.[2, 3]

Safety: what the labels warn about

Both labels list warnings that the treating team monitors. The boxed warning row in the table above is the most serious level of warning on a US label.

  • Itovebi: common side effects listed on the label include low neutrophils, low hemoglobin, raised fasting glucose, low platelets, mouth sores and diarrhea.[2]
  • Orserdu: common side effects listed on the label include musculoskeletal pain, nausea, raised cholesterol, raised liver enzymes (AST), raised triglycerides and fatigue.[3]

How families in the Gulf can access Itovebi or Orserdu

If the medicine is listed in your country. Ask your specialist and the hospital pharmacy about local supply first. A listing does not mean every hospital stocks it, but local supply is usually the fastest route and you may not need us.

If it is not registered. The usual legal route is a named-patient request: the treating hospital, on your specialist's written request, asks the national regulator for permission to import the medicine for one patient, using the US FDA approval as the reference. In Saudi Arabia the SFDA Special Access Program guidance covers these requests; in Bahrain, NHRA Circular 41 of 2020 sets the procedure. Other Gulf countries have their own procedures, which the hospital pharmacy confirms.[6, 7] Once approval is in place, Reserve Meds arranges supply through a US-licensed pharmaceutical wholesale partner to the hospital pharmacy.

We give you a written timeline estimate once we see the hospital file. The regulator, the treatment center and the manufacturer set their own timelines.
