About non-small cell lung cancer and these two medicines
Lung cancer forms in tissues of the lung, usually in the cells that line the air passages. There are two main types, small cell and non-small cell lung cancer, and non-small cell lung cancer is the more common type.[1]
Treatment depends partly on whether the cancer has changes in certain genes, such as EGFR or ALK. Your doctor may suggest biomarker tests to help predict the response to certain targeted therapy drugs.[2]
The main difference
These are not alternatives. Keytruda is an immunotherapy and Pemrydi RTU is a solution of the chemotherapy pemetrexed that is given without further dilution. For first-line metastatic nonsquamous NSCLC without EGFR or ALK changes, both labels describe giving them together with platinum chemotherapy, on the same day, Keytruda first.[3, 4]
The evidence behind each approval
Keytruda. KEYNOTE-189 was a randomized, double-blind trial in 616 patients with untreated metastatic nonsquamous NSCLC without EGFR or ALK changes. Keytruda plus pemetrexed and platinum chemotherapy was compared with placebo plus the same chemotherapy, and the label reports a statistically significant improvement in overall survival and progression-free survival.[3]
Pemrydi RTU. The Pemrydi RTU label describes the same pemetrexed trials, including KEYNOTE-189. Pemrydi RTU itself was approved on 22 May 2023 (Drugs@FDA, NDA 215179). Pemetrexed was first approved in the US in 2004.[4, 5]
One trial, two medicines. KEYNOTE-189 appears in both labels because it tested Keytruda added to pemetrexed and platinum chemotherapy. It does not compare Keytruda with pemetrexed; it shows the combination against chemotherapy alone.
How the two are used together
In KEYNOTE-189, Keytruda, pemetrexed and cisplatin or carboplatin were given on day 1 of each 21-day cycle for 4 cycles, then Keytruda and pemetrexed continued every 3 weeks. Pemrydi RTU is one pemetrexed product; the hospital may use another pemetrexed brand that is registered locally.[3, 4]
Safety: what the labels warn about
Both labels list warnings that the treating team monitors. The boxed warning row in the table above is the most serious level of warning on a US label.
- Keytruda: common side effects listed on the label include fatigue, musculoskeletal pain, rash, diarrhea, fever, cough and decreased appetite (when used alone).[3]
- Pemrydi RTU: common side effects listed on the label include fatigue, nausea and loss of appetite (when used alone).[4]

How families in the Gulf can access Keytruda or Pemrydi RTU
If the medicine is listed in your country. Ask your specialist and the hospital pharmacy about local supply first. A listing does not mean every hospital stocks it, but local supply is usually the fastest route and you may not need us.
If it is not registered. The usual legal route is a named-patient request: the treating hospital, on your specialist's written request, asks the national regulator for permission to import the medicine for one patient, using the US FDA approval as the reference. In Saudi Arabia the SFDA Special Access Program guidance covers these requests; in Bahrain, NHRA Circular 41 of 2020 sets the procedure. Other Gulf countries have their own procedures, which the hospital pharmacy confirms.[8, 9] Once approval is in place, Reserve Meds arranges supply through a US-licensed pharmaceutical wholesale partner to the hospital pharmacy.
We give you a written timeline estimate once we see the hospital file. The regulator, the treatment center and the manufacturer set their own timelines.
