What Jascayd is
Jascayd (nerandomilast) is a tablet made by Boehringer Ingelheim. It is a phosphodiesterase 4B (PDE4B) inhibitor. The FDA approved it on 7 October 2025 for idiopathic pulmonary fibrosis (IPF) in adults, and in December 2025 for progressive pulmonary fibrosis (PPF) in adults. The pivotal trial for PPF was FIBRONEER-ILD.
Pulmonary fibrosis is scarring of the lungs that makes breathing harder over time. Jascayd is a long-term treatment that a lung specialist starts and follows up.
How it is given and safety
The usual dose is 18 mg by mouth twice daily, about 12 hours apart, with or without food. The tablet can be swallowed whole or dispersed in water. The dose is lowered to 9 mg twice daily for people who cannot tolerate 18 mg (but not for people also taking pirfenidone, who stay on 18 mg) and for people taking strong CYP3A inhibitors. Tablets come in 9 mg and 18 mg strengths.
The US label lists no contraindications. Common side effects reported by Boehringer Ingelheim include diarrhea, COVID-19, upper respiratory infections, depression, weight loss, decreased appetite, nausea, tiredness, headache, vomiting, back pain and dizziness. Diarrhea was more frequent in people also taking nintedanib, and it was the most common reason to stop treatment.
How access works in Saudi Arabia
Because Jascayd is approved by the US FDA, a licensed Saudi hospital can ask SFDA for permission to import it for one named patient. This is often called the named-patient route.
SFDA expects the medicine to be registered with at least one reference regulator, such as the US FDA or the European Medicines Agency. The hospital signs a declaration that it takes responsibility for safe use. A licensed importer signs a declaration covering quality during transport. The patient or a legal guardian gives written consent after the treating doctor explains the benefits and risks.
SFDA's Special Access Program is a different route, for medicines with no reference approval anywhere, such as products still in clinical trials. Jascayd does not need that route because it already has FDA approval.
Public lists can lag behind registration decisions, so the hospital pharmacy confirms the status in SFDA's system before anything is filed.
Who does what
- Your treating specialist decides whether Jascayd is right for the patient and writes the clinical letter and prescription.
- The hospital files the import request, receives the medicine and dispenses it to the patient.
- The Saudi Food and Drug Authority (SFDA) registers medicines and decides on any import request for a medicine that is not registered.
- Boehringer Ingelheim makes Jascayd and decides where it is supplied.
- Reserve Meds helps your family understand the steps, organizes records and communication, and, where an import is approved, arranges US supply through our licensed wholesale partner. We do not give medical advice.
Documents you will usually need
- The lung specialist's report confirming IPF or PPF, with the high-resolution CT report
- Recent lung function test results
- A list of current medicines, especially pirfenidone, nintedanib or any strong CYP3A inhibitor, because they change the dose plan
- For an import request: the hospital's request form, its signed declaration of responsibility, written patient consent, and proof of the US FDA approval (we can supply the US prescribing information)
Steps
- Doctor's letter. Your specialist writes the clinical letter and prescription.
- Hospital request. The hospital pharmacy files the import request with SFDA.
- Regulator decision. SFDA reviews the request and approves it or asks for more information.
- US sourcing. Our US-licensed wholesale partner supplies the medicine, shipped with tracking.
- Hospital receipt. The hospital pharmacy receives the medicine and dispenses it to the patient.
